Our Mission is to help each individual in their home and local community reach their full potential and assist them in living as independently as possible. We strive to achieve this mission through the help of the members of our professional staff.

These are the words we live by for our individuals, their parents/guardians, and our staff. Throughout our history, we have continued to ask, “What can we do to encourage our individuals to reach their full potential as well as ensure their health and safety?” With collaboration, we have committed to doing just that. Of course, our role has evolved through the years, but we have continued to connect those with developmental disabilities and our direct service providers to provide the best quality of care.
We have a positive work culture of mentoring and leadership that flows from our administrative team down to our managers/team members and direct service providers. Our leadership team cares deeply and believes in quality of service for our individuals and their families. We believe in professionalism and demonstrating kindness and determination. We have an amazing, genuine and dedicated administrative team that leads by example with an emphasis on mentoring and servicing with compassion and understanding.
Your Home Health Agency was founded in 2008. In the beginning, we provided private pay home care services. We also provided home health aide staffing through a contractual relationship for Passport, Medicaid, and Medicare Certified Agencies. We no longer provide these services.
However, by 2011, we here at Your Home Health Agency expanded our services to become a Medicaid Certified Agency through the Ohio Department of Developmental Disabilities to provide Homemaker Personal Care, Non-Medical HPC Transportation, and Supported Living for individuals with Level 1, Individual Options Waivers, Self-Waiver, and Shared Living.
